This analysis of decision-making in the energy field, especially electricity in the UK and Western Europe, provides a critique of the various claims that are made for different ways of approaching the critical need for vast amounts of new investment. It explores reasons for the paralysis in decision-making, which is contrasted with past periods of rapid change, including the growth of electricity and renewables in the late 19th century, the development of centralised grids in the 1920s and 1930s and the rapid deployment of nuclear power in the 1970s and 1980s.

The UK faces a potential crisis in investment in its power infrastructure just as the need for new electricity-generating capacity is becoming urgent. Over the next decade or so the majority of its existing power stations are likely to have reached the end of their operating lives yet very little new reliable capacity is being built to replace it and a government decision to build new nuclear power stations announced in early 2008 had produced no fruit eight years later.

This book briefly reviews the history of each major fuel then considers the reasons for the decision sclerosis and suggests possible ways out of it. It reviews the difficulties of investing in long-term electricity generating capacity in competitive marketplaces; the problems caused by a lack of understanding of basic physics among elected representatives coupled with a degree of short-term political opportunism; the fundamentally dishonest approach of the Big Green industry; a naïve approach to communication on the part of the energy industry itself and a lax approach to scientific rigour among those advocating action against climate change which has bred disillusionment and suspicion. Comparisons with recent experience in Germany and Japan are offered to illustrate the issues.

This book is the first to cover the engineering aspects of severe plastic deformation (SPD) technology used to refine grain structure in metallic materials. The fundamentals of both the well-known and novel SPD processes are explained and the engineering know-how required for successful implementation of these processes revealed. The principles of each SPD technique are explained and insight provided into the mechanics of material deformation and microstructural changes. The equipment used for SPD processing is described including machines and tools.

The book covers the most popular SPD process of equal channel angular pressing, ECAP and its incremental version, I-ECAP. A separate chapter is devoted to tooling used in ECAP/I-ECAP. Another popular SPD process is high pressure torsion (HPT), which produces very good results in terms of refining grain structure but faces some technical challenges. A less known SPD process is cyclic extrusion compression (CEC), which is thoroughly explained as is twist extrusion (TE) which is a relatively new process that is showing good potential. Finally, an original SPD process of accumulated roll bonding (ARB), capable of refining grain structure in sheets, is discussed.

The book is intended for students and researchers working in the field of refining grain structure of metals by SPD. By explaining the engineering aspects of SPD, it enables the best SPD process to be chosen for a given application thus avoiding time-consuming and wasteful trials. It also encourages metal forming researchers and material scientists to work together in order to improve existing and develop new SPD processes. Finally, this book is also for industrial engineers, who will ultimately be using the SPD technology for mass production of metals with refined grain structure and improved properties.

Lignites are a fuel resource upon which there has been heavy reliance for a long time in several parts of the world. Indeed, lignite (also known as low-rank coal, brown coal or braunkohle), has been used for electricity generation in some regions for a century or more. These coals can, after a mild drying process, be used directly as a fuel and this remains the dominant form of usage. The coals can however be beneficiated in a number of ways including moulding into briquettes for export. Other new technologies applied to brown coals include slurrying and solar drying to make a hard product also suitable for export. Very importantly, over a period of 70+ years there has been hydrogenation of such coals to make liquid fuels.

This volume covers all aspects of the subject from the nature of lignites in situ to detailed coverage of fuel usage including figures for electricity generation and carbon dioxide release. Processing technologies including briquetting and carbonisation are described as are gasification, to make a fuel gas or a synthesis gas, and their conversion to liquid fuels. The book provides an international review, setting in context the use of lignite in various regions of the world.

Where appropriate the book includes information about industrial plant and processes and uses information from key research and development. It also considers the important issue of carbon dioxide emissions which in the past has sometimes worked against lignite utilisation. This issue is covered with some emphasis and also deals with carbon capture and sequestration from power plants. Co-firing of lignites with biomass is also considered.

This is the only recent comprehensive volume on the subject, bringing together for the first time a full account of this important fuel.

Managing the Myths of Health Care
Bridging the Separations between Care, Cure, Control, and Community
Health care is in desperate need of an overhaul: its costs have been rising for decades far faster than its results. In this sure-to-be controversial book, leading management scholar and iconoclast Henry Mintzberg turns his attention to reframing the management and organization of health care.

Mintzberg begins by describing various myths of health care, notably that this “system” is failing. It's hardly a system, but where it puts its effort, it is succeeding remarkably: we are living longer thanks to the many advances in treatments. But the current system is expensive, and we don't want to pay for it. So the administrators, in governments and insurance companies alike, have been intervening to fix it, mostly by cutting costs. And here is where we find a good deal of the failure.

The problem is not management per se but a form of remote-control management that has become too prevalent. Detached from the operations yet determined to control them, it reorganizes relentlessly, measures like mad, promotes a heroic form of leadership, favors competition where more cooperation is needed, and pretends that health care should be managed like a business.

There are also problems with how health care is organized. Every organization differentiates its work into component parts and then integrates these into unified wholes. But as Mintzberg points out, in health care, the predominance of differentiating over integrating has encouraged all sorts of excessive separations: “consulting” physicians who barely talk with one another, a preoccupation with evidence at the expense of experience and with researching cures for diseases while failing to investigate their causes, and the reducing of persons to “patients” and communities to “populations.”

Behind all this lies a professional form of organizing that is the source of health care's great strength as well as its debilitating weakness. Professionals, and often administrators as well, categorize whatever they can so they can apply standardized practices. When the categories fit, this approach works wonderfully well. When they do not—say, a “patient” falls between the categories—it fails. Even more damaging can be the misfit between managers and professionals, as they pass each other like ships in the night.

After the diagnosis, Mintzberg offers the remedy. He shows how the management of health care can be reframed by engaging more than detaching—that is, caring more than curing (heretically, like nursing). And management can be distributed beyond just those people called managers—for example, to facilitate the organic emergence of strategies from the base rather than having them handed down from “the top.” Health-care organizations can be reframed by encouraging collaboration to transcend competition and “communityship” to transcend leadership.

The overall message of Mintzberg's masterful analysis is that care, cure, control, and community have to work together, within health-care institutions and across them, to deliver quantity, quality, and equality simultaneously.
